Hospice Services Overview

Pharmcare’s long term care division – LTC Rx is committed to partner with our hospice providers and their Medical Directors to provide excellent care to our hospice patients. Our staffs are well trained in palliative care and have deep compassion for the patients and their families at the end of life. We respond quickly and efficiently when there is a change in condition by delivering relief and comfort medications to each patient’s location in a time of need. We are also aware of the challenges of controlling the cost at the same time providing quality care to our patients. Therefore, our pharmacists are always pro-active in suggesting alternative therapeutic equivalent to help control cost for our hospice providers.

Our Hospice Care Pharmacy program offers:

•  Medication Blister pack for facilities
• Bottle Pack for home patients
• Three daily Routine Delivery at 10AM, 1PM, and 6PM
• Hot Shot Anytime Delivery
• 24 Hour On-Call service – 7 days a week – 365 days a year
• Service covered areas for delivery include Conroe to Galveston, all Houston and surrounding areas including Baytown, Liberty, Dayton, Crosby, Bay City, Sealy, Katy, The Woodlands, Richmond and more
• UPS shipping is available for outside of delivery service area.  Any where in Texas is next day with ground shipping from Monday through Friday
• Pharmacist participates in Inter Disciplinary Team (IDT) meetings
• IV infusion, PCA CADD pump
• Hospice Prescription Compounding for end-of-life
• For convenience and time saving for the nurses and physicians, Pharmcare provides already made labels for CII triplicates for physicians to sign
• Competitive drugs prices and reasonable delivery fees
• Private and direct contract or through pharmacy benefits managers (PBM).
